The first mention of this village comes from a document issued in 1458, under which the brothers Stanisław and Jan Kmit made a division of the inheritance of the deceased brother Nicholas. The first mention of the Orthodox church in Piątkowa dates back to 1515. The village was mentioned only in 1552. In 1785, there were 1020 people living in Piątkowa - of which 173 were Roman Catholics (17.0%), 830 ounces (81.3%) and 17 Jews (1.7%). After passing several dozen meters, on the left side, among dense trees, we will see a wooden Greek-Catholic church. A hardened forest road leads towards it. In the place where the Orthodox church stands, there is the mouth of the right tributary of the Jawornik stream (from the south). The parish church of St. Dmitri was built in 1732 and renovated in 1881. It is one of the most beautiful churches in south-eastern Poland.

Wooden one with a log structure, three-piece, presbytery closed with a straight wall, wider aisle. The temple is crowned with three domes, the side walls are surrounded by a prominent roof supported by brick columns. In the years 1939-45 it was damaged. It was renovated and partially reconstructed in 1958-61. The church equipment is not preserved."
